How to get the most from this form.

Ask one thing at a time

Each question should address a single dimension. Mixing 'Was the product good and was the support helpful?' into one question makes responses ambiguous.

Include at least one open field

Structured ratings give you data. Open text fields give you insight. Always include one 'Anything else?' or 'Tell us more' field at the end.

Send it at the right moment

Timing matters. Send feedback requests immediately after a transaction, support interaction, or product milestone — not weeks later when the experience has faded.

Keep it under 2 minutes

Customers will complete short feedback forms. Anything that takes more than 2 minutes to fill out will have significantly lower completion rates.

What questions should a customer feedback form ask?
A well-structured customer feedback form should ask: (1) Overall satisfaction (radio or rating), (2) Likelihood to recommend (NPS-style), (3) What went well, (4) What could be improved. Keep it to 4–6 questions total.
How is a customer feedback form different from an NPS survey?
An NPS (Net Promoter Score) survey asks one specific question: 'How likely are you to recommend us?' on a 0–10 scale. A customer feedback form is broader — it captures satisfaction across multiple dimensions and includes open-ended responses.
