Field types

Every field Formiqa offers, grouped by purpose — what each one collects, and how to make fields required, validated, or conditional.

Every field shares the same basics: a label, an optional placeholder, a required toggle, and validation. Choice fields also let you define the list of options.

Input fields

Input fields collect a typed answer. Pick the one that matches the data so you get clean, structured responses:

  • Text — a single line of free text, for names, titles, or short answers.
  • Email — a single line that expects a valid email address.
  • Number — numeric input, for quantities, ages, or amounts.
  • Phone — a field for telephone numbers.
  • Long text — a multi-line box for messages, comments, or detailed answers.
  • Date — a date picker for birthdays, deadlines, or appointments.
  • Time — a time picker for hours and minutes.

Choice fields

Choice fields let respondents pick from options you define, which keeps answers consistent and easy to analyse:

  • Dropdown — a select menu where the respondent chooses one option from a list.
  • Single choice — radio buttons where exactly one option can be selected.
  • Multiple choice — checkboxes where the respondent can select several options at once.
  • Client picker — a searchable field tied to your saved Clients list, a lightweight contact list you keep in your dashboard, so respondents (or you) can pick an existing contact.

Advanced fields

Two fields go beyond typed and chosen answers:

  • File upload — lets respondents attach one or more files, such as a CV, a photo, or a document. See collect file uploads.
  • Signature — a canvas pad where the respondent draws an e-signature, for consent forms, agreements, or approvals.

Layout elements

Layout elements shape how the form reads. They are not questions — they add structure and context around your fields:

  • Heading — a section title that breaks a long form into clear parts.
  • Paragraph — a block of static explanatory text, for instructions or context.
  • Divider — a visual separator line between sections.
  • Page break — splits the form into multiple steps. See multi-step forms.

Making fields dynamic

Any field can be more than a static box. Use validation and required fields to enforce the format of an answer and mark it as mandatory, and use conditional logic to show or hide a field based on how earlier questions were answered.

Frequently asked questions

What's the difference between single choice and multiple choice?
Single choice uses radio buttons and allows exactly one selection — pick one option and the others clear. Multiple choice uses checkboxes and lets the respondent tick as many options as apply at the same time.
What is the client picker field?
It's a searchable field tied to your Clients list — a lightweight contact list kept in your dashboard. Instead of typing a name freehand, the respondent (or you) searches and selects an existing saved contact.
Can I make any field required?
Yes. Every field has a required toggle in its side editor. When it's on, the respondent cannot submit the form until they have answered that field.
Do headings and dividers show up in my responses?
No. Heading, paragraph, divider, and page break are layout elements that structure the form but collect no data, so they never appear as columns in your submissions.
